SNFIST Program
The SNFIST (Skilled Nursing Facility Inpatient Specialist) Program provides dedicated physician coverage inside skilled nursing facilities: ensuring residents receive the same quality and continuity of care as hospital inpatients.
Our SNFIST physicians conduct rounds, respond to acute changes in condition, manage complex medication regimens, and coordinate with the facility nursing team to prevent avoidable hospitalizations.
