We order labs, x-rays, and EKGs and coordinate portable x-ray and EKG services in your home, so you rarely have to leave. Lab work is completed at a convenient lab or during a clinic visit.
Care Coordination
We collaborate with home health agencies to arrange supplementary services: visiting nurses, physical therapy, wound care nursing, occupational therapy, hospice, and durable medical equipment.
Medication Management
Prescription management, refill coordination, and medication reconciliation at every visit. Our in-house pharmacist provides additional medication education and adherence support.

Urgent concerns after hours? Our providers are available around the clock for emergencies. Patients and families can reach a provider by phone at any time.
Remote Patient Monitoring
HPG's RPM program extends our home care with 24/7 vital sign monitoring -- blood pressure, cardiac rhythm, oxygen levels, and more -- alert-based and physician-reviewed.

Home visits are covered by:
