The GUIDE Program for Dementia Care
Guiding an Improved Dementia Experience (GUIDE) is a Centers for Medicare & Medicaid Services innovation model launched in 2024. It delivers comprehensive, coordinated care for people living with dementia and critical support for their caregivers.
What Is the GUIDE Model?
The GUIDE Model is a voluntary, nationwide alternative payment model from the Centers for Medicare & Medicaid Services (CMS). It aims to improve quality of life for people with dementia while reducing caregiver strain and unnecessary hospitalizations.
Home Physicians Group is a participating GUIDE Model Participant, bringing this program to elderly patients and their families across Central Florida. Our physician-led teams provide coordinated dementia care at home, in assisted living communities, and in skilled nursing facilities.

The GUIDE Program is available to Medicare beneficiaries who meet the following criteria:
Dementia Diagnosis
Patient has been diagnosed with Alzheimer's disease or a related dementia by a qualified clinician.
Medicare Enrolled
Must be enrolled in traditional Medicare (Parts A & B). Not enrolled in a Medicare Advantage plan or Hospice.
Central Florida Resident
Living in our service area across 14 Central Florida counties, including in a home, ALF, or SNF setting.
Caregiver Involvement
A caregiver, family member, or support person is available to participate in care coordination and planning.
The Five Pillars of GUIDE Care
Our GUIDE program delivers evidence-based support across five essential areas:
Care Navigation
A dedicated Care Navigator coordinates all medical appointments, specialist referrals, community resources, and benefits so families spend less time managing logistics.
Comprehensive Care Planning
Individualized dementia care plans covering medication management, behavioral symptoms, safety assessments, advance care planning, and goals of care discussions.
Caregiver Support
Education, training, and emotional support for caregivers. Respite services help prevent caregiver burnout, a critical component often missing from traditional care models.
24/7 Crisis Support
A 24/7 helpline staffed by trained dementia care professionals, available for urgent clinical questions, behavioral crises, or reassurance at any hour.
Community Resource Linkage
We connect patients and families with local support groups, adult day programs, legal and financial planning resources, and community services that complement medical care.

GUIDE Program FAQ
GUIDE Program services are covered under traditional Medicare at no additional cost to the patient beyond normal Medicare cost-sharing. You do not need a supplemental plan, though one may further reduce out-of-pocket costs.
Within the GUIDE model at HPG, our providers serve as the coordinating care team. We work collaboratively with any existing specialists to ensure integrated care. Contact us to discuss your specific situation.
The GUIDE Program covers all types of dementia, including Alzheimer's disease, vascular dementia, Lewy body dementia, frontotemporal dementia, and mixed dementia, as long as the diagnosis is documented and the patient meets Medicare eligibility requirements.
GUIDE services can be provided wherever the patient lives, including private homes, assisted living facilities, and skilled nursing facilities. HPG's mobile care model means the care team comes to your loved one.
