Absolutely, that is one of our specialized services. If you or your loved one is homebound, we will provide medical care in the comfort of your home. Our providers come to you, whether you are recovering from surgery, managing a chronic condition, or simply unable to travel to a clinic.
Our medical providers can order labs, x-rays, EKGs, and other diagnostic services. In most cases, you almost never have to leave your home for these to be done. We coordinate with home diagnostic services to bring testing directly to you.
Yes. Medicare and many other insurance plans provide coverage for services such as a visiting nurse, physical therapy, wound care nursing, and durable medical equipment. HPG coordinates with home health agencies to arrange these supplementary services as part of your care plan.
We assign a carefully selected home care team of providers to ensure quality care and continuity. While the exact clinician may vary between settings, your entire care record and plan follow you. The team always has full context of your history.
Our Advanced Registered Nurse Practitioners (ARNPs) make most regular visits. They hold Master's of Nursing degrees and ARNP certifications and are fully qualified to manage your care independently. Physicians oversee all care plans and are involved whenever clinically indicated.
Many of our team members are bilingual. Spanish-speaking staff are available, and patient registration forms, consent documents, and other materials are offered in Spanish as well. Hablamos español.

For homebound patients, we accept Medicare and some PPO plans. For skilled nursing facility patients, we accept most major medical plans. Our accepted insurances include Medicare, Florida Medicaid, Aetna, Humana, UnitedHealthcare, Cigna, Blue Cross Blue Shield of Florida, and Florida Hospital plans. We also have active managed care contracts with Humana, Aetna, and Wellcare MC. Contact us to verify your specific plan.
We offer competitive pricing and accept most major insurance plans. Financial assistance programs may be available. Please call our office to discuss your specific situation. We will work to find a solution that works for you.
Our medical providers can prescribe medications and order refills. Please note that most medication requests require 24 to 48 hours to process. Our in-house pharmacist also provides medication education, adherence support, and can help manage complex drug therapy regimens.
HPG provides a comprehensive ecosystem of services: primary care (home visits and clinic), SNFIST program (skilled nursing facilities), transitional care (hospital to home), chronic care management, remote patient monitoring, the GUIDE dementia program, psychiatry, podiatry, pulmonary care, wound care, pharmacist services, chaplain services, case management, weight loss program, and more, all under one coordinated team.
Our main office is at 12301 Lake Underhill Rd Suite 215, Orlando, FL 32825. We also operate a clinic in DeLand at 450 N McDonald Ave, DeLand, FL 32724. However, HPG is designed to be ambulatory. Our providers come to you, so you rarely need to visit an office.
