My name is Dr. Gabriel Nuriel, and I founded Home Physicians Group more than 20 years ago with a straightforward belief: every patient deserves access to quality medical care, regardless of where they live or how well they can travel. Whether you are a current patient, a family caregiver, or someone exploring care options for a loved one in Central Florida, this space was created for you.
A message from Dr. Nuriel
Before you read further, I want to share a brief video introduction. In it, I talk about who we are at HPG, why I built this practice the way I did, and what we believe medicine should look like for patients who cannot easily leave their homes.

Who we are and how we work
Home Physicians Group is a physician-led practice built around one core idea: the quality of your medical care should not depend on your ability to drive to a clinic. Our team of physicians, nurse practitioners, and physician assistants provides primary care to adults of all ages across Orange, Osceola, Seminole, Volusia, and 10 additional Central Florida counties. While much of our work supports seniors and homebound patients, we welcome adult patients at every stage of life, whether you are managing a chronic condition, recovering from a hospital stay, or simply looking for a primary care provider who has time for you.
We go where our patients are. That includes private homes, assisted living facilities, and skilled nursing facilities. We also operate two clinic locations, in Orlando and DeLand, and we provide telehealth visits for patients who prefer to be seen from the comfort of home. We are not a visiting nurse service or a home health agency. We are a full-scope primary care practice that meets you wherever you are: at home, in our clinics, or by video.
Our programs include Mobile Primary Care, the GUIDE Program for dementia care, Remote Patient Monitoring, Transitional Care after hospitalization, and Chronic Care Management for patients living with multiple chronic conditions. Each program is designed to keep patients safer at home and out of the hospital.
Medicare coverage: Most of our primary care services are covered by Medicare Part B, including home visits, office and telemedicine visits, chronic care management, and remote patient monitoring. Call 321-235-0692 to confirm coverage for your specific plan.

Our goal is to publish content that is genuinely useful: not generic health information, but physician-authored guidance written for the patients and families we actually serve in clinical practice. Here is what we plan to cover:

We will also address the questions we hear most often in clinical practice: how to choose between a clinic, home care, or assisted living; when to consider transitional care after a hospital stay; and how to navigate the healthcare system when a loved one can no longer manage it alone.
Home Physicians Group serves adults of all ages across Central Florida, from younger adults seeking a primary care provider to seniors and homebound patients with complex needs. We deliver care at home, in our Orlando and DeLand clinics, and by telehealth.

Dr. Gabriel Nuriel is a family physician who received his undergraduate degree from Oral Roberts University and his medical degree from Nova Southeastern University. He served as Chief Intern and Chief Resident at Florida Hospital during his residency training.
Dr. Nuriel spent four years as an Emergency Room physician at Florida Hospital and served as Medical Director of the Healthcare Center for the Homeless in Orlando. He founded Home Physicians Group more than 20 years ago and has grown the practice into a physician-led ecosystem serving patients across 14 Central Florida counties.
He is a member of the Florida Osteopathic Medical Association (FOMA) and the American Osteopathic Association (AOA).
